  3. COUNTRY/LOCATION
  4. FULL DESCRIPTION OF ERROR
  5. UNIT/PART ITEM NUMBER
  6. HOUR COUNTER VALUE
  7. WHAT ACTIONS HAVE BEEN TAKEN TO SOLVE THE ISSUE
  8. HOW FREQUENT DOES THE ISSUE OCCUR
  9. WHEN DOES THE ISSUE OCCUR: During startup/during normal operations/other
  10. CURRENT SOFTWARE VERSION
  11. IS THE SOFTWARE REGISTERED IN COTES SYSTEM, VIA QR-CODE: Yes/No
